Medicare Advantage revenue now turns on the scoring rulebook. CMS's 2027 star ratings could move billions between insurers with no change in membership.
CMS released 2027 Medicare Advantage star ratings on Oct 8. Humana now has 93% of members in plans rated four stars or more, per Healthcare Dive. TD Cowen estimates $3B or more of extra 2028 revenue. Humana stock rose about 15% on Oct 9 while CVS fell about 3%. Aetna said 69% of its members are in four star plans.
Alignment Healthcare's largest contract fell below four stars, cutting its four star share to 25% from 98%, and its shares fell more than 20%. The average rating slipped to 3.99 from 4.01, and five star contracts fell to 15 from 22. Clover Health reached 100% after a court set aside parts of the method behind its 2026 scores.
The layer moving is regulation. CMS dropped 11 administrative measures and raised thresholds, so bonus dollars follow measured outcomes and court rulings, not scale. Profit Migration governs but stays Inactive: one year of scores is not a margin shift in filings. Confidence is Medium: the ratings are public, but revenue effects are analyst estimates.
