Traditional Medicare now competes with Medicare Advantage on cash. CMS will pay $90 to 20.8M fee for service enrollees and nothing to MA members.
CMS published terms on Oct 3 for a one time $90 Part B premium rebate from the Medicare Improvement Fund, and the President announced it Oct 4. It goes to 20.8M Original Medicare enrollees by direct deposit from about Oct 8. Medicare Advantage members are excluded, as are Medicaid premium aid recipients and IRMAA payers. Open enrollment starts Oct 15. That implies about $1.9B by our math, not a CMS figure.
Federal payment friction is also in view. On Oct 1 CMS met all 17 certified No Surprises Act arbiters. Over 7 million disputes have been received and closed since April 2022, costs are rising quickly, and a new IDR Gateway arrives in late 2026.
The layer moving is capital: Medicare money paid to patients directly, bypassing plans. Profit Migration governs if it shifts enrollment from MA. Confidence is Low: $90 once is small, legal authority is untested, and enrollment effects are unknown.

WHAT WOULD PROVE THIS WRONG
CMS reports Medicare Advantage share of new 2027 enrollment at or above 2026 levels, or a court or Congress blocks the rebate before most payments go out in October 2026.
