Drug discounts are moving onto a claims data platform drugmakers control. HRSA cleared 10 manufacturers, all on Beacon, for 340B rebates from Jan 1.
HRSA approved 10 drugmakers for its revised 340B rebate pilot, reported Oct 1 and Oct 2, covering 21 drugs including Eliquis, Farxiga and Jardiance. From Jan 1, 2027, covered entities pay full price first, then submit claims level data within 45 days, and manufacturers pay or deny within 10 days. All 10 use the Beacon platform, and manufacturers pay its costs. The drugs are under 5.5% of 2025 program sales. A court blocked the first pilot.
Claims data is also the center of enforcement. Independence Blue Cross agreed on Oct 2 to pay $22.5M to settle allegations that it submitted inflated Medicare Advantage diagnosis codes from 2016 to 2020.
The layer moving is infrastructure. Whoever runs the data platform decides when a discount is earned. Distribution Capture governs. Confidence is Low: the pilot is small, hospitals oppose it, and courts have blocked it before.
